It’s that time of year again when we open up the Health Insurance Opt Out Program. Attached to this message are the program guidelines and form. Please review this important information.

A few things to keep in mind:

Forms must be submitted electronically to School Human Resources no later than Friday, November 22, 2024, at 5 PM in order to receive the buyout
All documentation must be included with the form or the request will be denied
Forms must be completed in full. If not, the payment will be delayed
When submitting the form, you must provide a letter or documentation from the health insurance carrier stating that you have active coverage in November 2024. This documentation must also provide the primary policyholder’s name. If you are not the primary policy holder, you are not eligible for a family plan. Providing an insurance ID card alone is not sufficient.
If you submit your forms after November 22, 2024, your opt out will not be processed.
